About the role:
As a care navigator at Tall Tree, you are the key contact for each patient in their healthcare journey. You help patients find the best care for their issue, connect them with the right clinician and service, and provide ongoing support with the rest of the team to help them achieve their goals. You'll leverage your deep knowledge about our services, and external services and resources to ensure every patient gets the support they need.

Care coordinators are experts in transforming potential patients into booked patients; You make the patient experience delightful, effective, easy, and consistent - you are the glue that connects the administrative and clinical aspects of care! Ultimately, you facilitate Tall Tree’s mission to do healthcare the way it should be done.

As a Care Navigator at Tall Tree, you will be focused on:

Patient Guidance and Support: you’ll build relationships with patients, matching them with the best-fit clinicians and services within Tall Tree or external resources, introducing available programs, and providing education to empower active participation in their health journey.

Care Coordination and Engagement: you’ll collaborate with clinical teams to create and support comprehensive treatment plans, ensure appointments and follow-ups are completed, and track progress and outcomes to deliver a cohesive healthcare experience.

Administrative and Resource Management: you’ll assist patients with funding options, manage referrals, and provide seamless communication between patients and the clinical team to support effective care delivery.

Delivering Exceptional Service: you’ll welcome patients warmly and professionally, answer questions about services and staff, and go above and beyond to meet their needs, creating moments that surprise and delight.

Proactive Problem Solving: You’ll address patient inquiries and concerns promptly, resolve complaints with timely and thoughtful decisions, and collaborate with the Office Manager as needed, demonstrating creativity and independence in problem-solving.

Scheduling and Appointment Management: You’ll manage patient appointments using Jane EMR, optimize clinical schedules, coordinate waitlists and cancellations, and ensure patients are booked for their full treatment plans. You’ll take ownership of proactively communicating with patients regarding funding eligibility and making necessary scheduling adjustments.

Funding, Billing, and Accounts Receivables: You’ll submit health claims to insurers, manage patient billing and payments, liaise with funders and case managers, and ensure timely follow-up on outstanding payments. You’ll help support clinicians with treatment plan deadlines and funding submissions while maintaining clear communication with patients about funding statuses.

We’re looking for:

Relevant education: undegraduate training in a healthcare domain is an asset!

Proficiency with computers and software including Google Workspace, Microsoft Office, and slack

Experience with Jane Practice Management or similar EMR software.

Someone who thrives in a fast-paced environment. We’re looking for someone able and willing to go above and beyond to perform essential functions and manage multiple competing demands with grace and ease.

Interest and passion for working within a team, collaborating and engaging with a diverse range of professionals, and contributing to an exceptional work environment are a must.

Two years of recent related experience in a healthcare setting, or an equivalent combination of education, training, and experience. Experience working in an integrated healthcare setting is an asset.

Awareness and understanding of the different options available to patients seeking care from allied health professionals.

Customer service or sales experience (eg. sales and marketing, account management, or non-formal sales roles such as in the retail and services industries)

About the Role: Speech and Language Pathologist (SLP)

As a Speech and Language Pathologist at Sprout Family Health, you’ll be part of a multidisciplinary team redefining family-centered proactive healthcare. Working alongside a team led by a Nurse Practitioner and supported by a Health Navigator (Registered Nurse), as well as specialists in physical, functional, and mental health, you’ll deliver personalized, holistic care that supports the health and wellness goals of our families.

This role is focused on providing screening, parent education, and professional guidance. In this position, you will work with families to identify early signs of speech, language, or communication difficulties through screenings, offering informed recommendations and guidance on whether further treatment may be beneficial. Additionally, you’ll empower parents with education and resources to support their child’s communication development. When intervention is indicated, you will facilitate referrals to community-based SLPs for comprehensive assessment and therapy. Ideal candidates will have strong interpersonal skills, a collaborative approach, and a passion for supporting families through education and informed recommendations.

This is a permanent, part-time, contract role which is primarily virtual and occasionally requires commuting to our clinic in James Bay, Victoria BC.

Responsibilities:

Conduct Evaluations and Develop Treatment Plans: Assess patients’ communication skills, creating individualized plans for a wide range of conditions, including phonological disorders, aphasia, fluency disorders, apraxia of speech, articulation issues, language delays, cognitive-communication challenges, and voice disorders.
Deliver Patient-Centered, Evidence-Based Care: Provide high-quality, patient-centered care that’s informed by current research and best practices, educating patients and their families on effective communication strategies.
Collaborate Across Disciplines: Work closely with an interdisciplinary team that includes Nurse Practitioners, Registered Nurses, Occupational Therapists, Physiotherapists, Clinical Counselors, and other health professionals to create holistic, integrated care plans.
Coordinate with Stakeholders: Collaborate with third-party stakeholders, maintaining comprehensive reports and records in line with standards.
Pursue Professional Development: Stay current with industry developments, attending educational opportunities at Sprout Family Health and continuously refining skills to ensure the highest quality of care.

Qualifications:

Credentials: Current registration with the College of Health and Care Professionals of BC (CHCPBC) and a Master’s Degree in Speech Language Pathology, or equivalent from a recognized university.
Clinical Expertise: Strong foundation in evidence-based practice, with at least 1 year of clinical experience, ideally in Pediatric Speech Pathology focused on language disorders, articulation, and fluency.
